REMINDER: It is important to elect a beneficiary for your NYCDCC Annuity Fund account with Prudential and to update your beneficiary any time your financial situation changes. Updating your beneficiary ensures that should you die, your plan assets will go exactly where you want. If you don’t have a current beneficiary on file with Prudential, the Plan (per the rules of the Summary Plan Description), or the courts will make that critical decision for you. Wouldn’t you rather make that decision for yourself?

If you have any questions regarding your beneficiary for your account with Prudential, call 877-PRU-2100 (877-778-2100) weekdays from 8 a.m. to 9 p.m. ET and say “representative” at the prompt. To check or change your beneficiary for your Annuity benefits, you can contact Prudential for a beneficiary form by phone or visit their website at www.prudential.com/nycdccbf.

Please be aware that the Prudential form is separate from the beneficiary form you may have previously filled out from the Benefit Funds for the Welfare and Pension Fund.

Note: If you’re married and want to name someone other than your spouse as your primary beneficiary, you must submit a notarized copy of the spousal consent waiver (found on the beneficiary form).
